The play is set up with a vertical stack of 5 tight to the back of the endzone. One cutter is set in the middle of the endzone, in front of the stack. This leaves the cutter with lots of space to get free of their man.
The cutter uses the space to get free in a one-on-one matchup. There are many ways to get free from this position, but the handler must be able to make the throw when necessary. In this set up you would place your strongest thrower on the disc, and your strongest cutter in the ISO. The cutter forces their marker into the open side space...
and makes a cut across the endzone towards the break side for the score. This offensive play works by giving the cutter space to make the play. The rest of the stack are out of the way, keeping their defenders engaged on them, and not in the cutting space.

If the iso doesn't get free and the handler looks them off, they can swing to the other handler...
While the iso cutter rejoins the stack at the front.
The iso has failed and you return to a basic endzone play. Cut's start second from back...
